Lifted Hospice is seeking a facility-based Certified Nursing Assistant (CNA) to provide consistent personal care for hospice patients in a single location. You will support mobility, observe patient changes, and report to the RN while maintaining dignity and trust. Strong communication and a TX CNA credential are essential.

You will work in a caring, team-oriented environment with regular staff collaboration, safety protocols, and a commitment to compassionate end-of-life care.

Qualifications

• Completion of a state-approved CNA training program.
• Active CNA certification in the State of Texas.
• High school diploma or equivalent required.
• At least one year of experience in hospice, home health, or a similar care setting is preferred.
• Compassionate presence and strong communication skills.

Responsabilités

• Following the Plan of Care – Providing direct personal care under the supervision of the RN, in alignment with the CNA Plan of Care and patient preferences.
• Assisting with mobility and safety – Helping patients transfer, move, and ambulate safely — while maintaining a calm, clean, and secure environment.
• Observing and reporting – Noticing and promptly reporting any changes in the patient’s condition or behavior to the RN, supporting proactive care.
• Practicing infection control – Following all safety and hygiene protocols to protect the health of patients, families, and teammates.
• Respecting privacy and dignity – Upholding patient confidentiality, respecting personal property, and communicating with kindness and professionalism.
