Job summary

Lifted Hospice is seeking a compassionate Certified Nursing Assistant to join our field-based care team in Texas. You’ll provide essential personal care to patients in their homes and work under the supervision of a registered nurse.

A valid Texas CNA certification, a state-approved training program, and a high school diploma are required, with preference for hospice or home health experience. Strong communication, reliability, and the ability to lift up to 50 pounds are essential.

Qualifications

• Completion of a state-approved Certified Nursing Assistant (CNA) training program.
• Active CNA certification in the State of Texas.
• High school diploma or equivalent required.
• At least one year of experience in hospice, home health, or a similar care setting is preferred.

Responsibilities

• Providing direct personal care under the supervision of the RN, in alignment with the CNA Plan of Care.
• Assisting with mobility and safety – transferring, moving, and ambulating safely.
• Observing and reporting any changes in patient condition to the RN.
• Practicing infection control according to safety protocols.
• Upholding patient privacy and dignity with kindness and professionalism.
