Description
The Lifted Difference
At Lifted Hospice, our mission is simple yet powerful: Helping people live life to the fullest . We serve the communities where we live and work by delivering compassionate, clinically excellent end-of-life care. With a commitment to dignity, comfort, and quality of life, we walk alongside patients and their families—meeting them where they are, when they need it most.
What makes us different is how we show up. We care for patients and families like they’re our own—and we support each other the same way. At Lifted Hospice, you’ll be part of a culture built on trust, unity, and joy. We serve from a place of favor, and we never forget the sacredness of what we do.
At Lifted Hospice, our Certified Nursing Assistants (CNA) are more than caregivers — they are the hands and heart of compassionate, dignified care. In hospice care your reliability and compassion have lasting impact. A s a facility-based CNA, you’ll provide consistent personal care to hospice patients housed in one designated facility. You’ll be a steady presence—building trust with staff, patients, and families—while delivering high-quality care under the supervision of a registered nurse. Unlike field CNAs, this position is not mobile; you’ll serve patients in a single location, helping ensure comfort, dignity, and support every day.
As a CNA at Lifted Hospice, you play a crucial role in bringing dignity, comfort, and consistency to our patients and their families. You are often the most present member of the care team — the one who helps a patient feel seen, safe, and supported every day.
In this role, you’ll serve patients in facilities they call home — assisted living, nursing facility, or memory care setting.
You’ll provide personal care in varied environments, both indoors and outdoors, and may be exposed to infectious diseases and bodily fluids as part of your essential work. You’ll also regularly connect with your local care team to stay aligned on patient needs and ensure care is consistent, respectful, and personalized.
At Lifted Hospice, we don’t just work shifts — we carry the mission forward together.
