Job summary

Lifted Hospice is seeking a field-based Certified Nursing Assistant to provide direct personal care to patients in their homes across Texas. You’ll work under RN supervision, following individualized plans of care and prioritizing patient comfort and dignity.

Ideal candidates have a Texas CNA certification, at least one year in hospice or home health, and strong communication in English. The role requires reliable transportation and a willingness to travel for visits.

Qualifications

• Completion of a state-approved CNA training program.
• Active Texas CNA certification.
• High school diploma or equivalent.
• 1+ year hospice/home health experience preferred.
• English literacy required.
• Valid Texas driver’s license and reliable transportation.

Responsibilities

• Following the Plan of Care – Providing direct personal care under the supervision of the RN, in alignment with the CNA Plan of Care and patient preferences.
• Assisting with mobility and safety – Helping patients transfer, move, and ambulate safely — while maintaining a calm, clean, and secure environment.
• Observing and reporting – Noticing and promptly reporting any changes in the patient’s condition or behavior to the RN, supporting proactive care.
• Practicing infection control – Following all safety and hygiene protocols to protect the health of patients, families, and teammates.
• Respecting privacy and dignity – Upholding patient confidentiality, respecting personal property, and communicating with kindness and professionalism.
