Zusammenfassung

Lifted Hospice is seeking a field-based Certified Nursing Assistant (CNA) to provide essential personal care to patients in their homes or care settings. You will work under RN supervision, delivering compassionate, dignified care and supporting families.

Applicants should hold a Texas CNA certification, have a high school diploma or equivalent, and preferably 1 year of hospice or home health experience. A valid Texas driver’s license and reliable transportation are required.

Qualifikationen

• Completion of a state-approved CNA training program.
• Active CNA certification in the State of Texas.
• High school diploma or equivalent required.
• At least one year of experience in hospice, home health, or a similar care setting is preferred.

Aufgaben

• Following the Plan of Care - Providing direct personal care under the supervision of the RN, in alignment with the CNA Plan of Care and patient preferences.
• Assisting with mobility and safety - Helping patients transfer, move, and ambulate safely - while maintaining a calm, clean, and secure environment.
• Observing and reporting - Noticing and promptly reporting any changes in the patient's condition or behavior to the RN, supporting proactive care.
• Practicing infection control - Following all safety and hygiene protocols to protect the health of patients, families, and teammates.
• Respecting privacy and dignity - Upholding patient confidentiality, respecting personal property, and communicating with kindness and professionalism.
