Job summary

Soleo Health seeks a Full-time Infusion Nurse to support infusion patients in Englewood, CO and nearby areas. This role performs assessments, care planning, and education for home infusion therapy, collaborating with care teams to ensure safe, effective treatment.

Responsibilities include coordinating with pharmacists and prescribers, educating patients, and documenting care per policy. Must hold an active Colorado RN license and be willing to travel to patient homes.

Qualifications

• Active RN license in Colorado in good standing.
• BSN preferred but not required.
• Experience in infusion therapy or home care is preferred.

Responsibilities

• Provide direct care for infusion patients at home or alternate sites.
• Coordinate care with pharmacists, prescribers and care partners.
• Educate patients and care partners on home infusion therapy.
• Maintain accurate documentation per policy and state guidelines.
• Participate in nurse on-call rotation and ensure timely POT submission.
