Job summary

ABL Health Care in San Mateo, CA is seeking a compassionate, organized RN to join our Home Health team on a full-time basis. You will evaluate patients, create and implement care plans, and coordinate with the interdisciplinary team to ensure high-quality home-based care.

Qualifications

• Current Registered Nurse license to practice in California (CA).
• Experience in Home Health or Hospice care desired.
• Valid California driver's license and ability to travel.

Responsibilities

• Provide nursing care within the scope of practice, following the patient's individualized care plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or Director of Nursing.
• Maintain accurate clinical records and submit documentation using a company-provided device.
• Supervise Licensed Vocational Nurses (LVNs) when assigned.
• Comply with all company policies and procedures to ensure high standards of care
