Job summary

Aveanna Healthcare in Alabama seeks a dedicated Occupational Therapist (OT) to assess, teach, and deliver home health therapy under physician orders. You will collaborate with an interdisciplinary team and support patients and families to improve daily functioning and independence.

This full-time role runs Monday through Friday in Morgan and Limestone territories, with emphasis on accurate documentation, OASIS readiness, and discharge planning while maintaining compassionate, patient-centered

Qualifications

• Must hold a degree in Occupational Therapy from an accredited university/program.
• Current, unrestricted state license as an Occupational Therapist in the state of practice.
• Valid CPR certification.

Responsibilities

• Perform initial and ongoing assessments of mobility, coordination, strength and functional level.
• Deliver OT services per physician orders and plan of care.
• Document performance, adjust goals, and plan discharge.
• Discharge planning and home exercise program education for patients and caretakers.
• Coordinate with health care team, physicians, nurses and families.
• Complete OASIS assessments where allowed by state practice.
