Job summary

Adaptive Home Health, LLC seeks a Physical Therapist Assistant (PTA) to deliver skilled PTA visits in patients' homes across Texas under a supervising PT. This full-time, field-based role emphasizes mobility, safety, and practical rehab planning.

You will be paid on a productivity model, with 30 points per week at $58 per point, and scheduled Monday–Friday. Must hold Texas PTA license, valid driver's license and be comfortable daily travel.

Qualifications

• Active Texas PTA license in good standing.
• Strong clinical judgment and comfort working independently in the field (PTA scope with PT supervision).
• Comfort escalating changes in condition to PT/RN/MD with clear rationale.
• Strong communication and documentation.
• Reliable follow-through on treatment plans, visit cadence, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PTA scope.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on gait/balance, transfers, strengthening, and home safety.
• Set/track objective progress within PTA scope and communicate updates to the supervising PT.
• Drive multidisciplinary coordination and keep the care team moving.
• Coach patients and caregivers with practical home programs that get done.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
