Adaptive Home Health is seeking a Physical Therapist Assistant (PTA) for home health in Texas. This field-based, full-time role offers productivity-based compensation and requires a Texas PTA license, strong clinical judgment, and daily travel across the assigned territory.

You will deliver PTA visits under a supervising PT, focus on gait, balance, transfers, and home safety, and document promptly in Homecare Homebase. Schedule includes weekdays with occasional weekend work as needed.

Qualifications

• Active Texas PTA license in good standing.
• Strong clinical judgment and ability to work independently in the field.
• Comfort escalating condition changes to PT/RN/MD with clear rationale.
• Strong communication and thorough documentation.
• Reliable follow-through on treatment plans and visits.
• Valid driver’s license and daily travel across territory.
• Ability to carry/transport therapy equipment.
• Competency in core PTA home health scope: gait training, transfers, safety, strengthening.

Responsibilities

• Deliver PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on gait, transfers, strengthening, and safety.
• Set/track progress and communicate updates to the supervising PT.
• Coordinate with care team to escalate changes and keep visits coordinated.
• Coach patients and caregivers with practical home programs.
• Document promptly in Homecare Homebase (HCHB).
