Job summary

Adaptive Home Health is seeking a Physical Therapist Assistant (PTA) for field-based home visits in Texas. You will work under a supervising PT to restore function and improve home safety, with a productivity-based compensation model and mileage reimbursement.

You’ll deliver skilled PTA visits, coordinate with the care team, and coach patients and caregivers on practical home programs while documenting thoroughly.

Qualifications

• Active Texas PTA license (in good standing).
• Strong clinical judgment for field work.
• Comfort escalating changes to PT/RN/MD with clear rationale.
• Clear communication and documentation.
• Reliable follow-through on treatment plans and goals.
• Valid driver’s license and ability to travel daily.
• Ability to transport basic therapy equipment.

Responsibilities

• Deliver skilled PTA visits in the home under PT plan of care.
• Execute treatment plans focused on gait, balance, transfers, strengthening.
• Set/track progress within PTA scope and update supervising PT.
• Drive multidisciplinary coordination and keep care team aligned.
• Coach patients with practical home programs they can perform.
• Document visits promptly and thoroughly.
