Whole Life is hiring a Direct Support Professional in Trumbull, CT. The role focuses on providing person-centered care to adults with developmental and physical disabilities, assisting with daily activities, and promoting independence within a supportive team.

The position offers long-term opportunities, accompanying training, and benefits eligibility. You will work in a community-based setting across Connecticut with 31 homes and programs.

Qualifications

• Proof of high school diploma or GED equivalent is required.
• Ability to pass background checks and health screenings (TB test, Fit for Duty).
• Must be able to perform physical tasks including lifting up to 100 lbs.
• Must document care in English and communicate effectively with clients and staff.

Responsibilities

• Assist with Activities of Daily Living (ADLs): bathing, dressing, grooming, toileting, and meal preparation.
• Administer medications, monitor vital signs, and support medical appointments.
• Maintain a clean, safe living environment through light housekeeping and laundry.
• Engage clients in social activities and community outings to promote independence.
• Use mobility equipment with proper techniques and ensure safety.
• Document daily activities, observations, and incidents accurately.
• Maintain confidentiality per HIPAA regulations and implement individualized care plans.
