Job summary

Community Health Resources, Inc. CHR is seeking an Outpatient Therapist for the Children’s Outpatient Program in Willimantic, CT, with duties split between Child Outpatient and Killingly Memorial School. The role is full-time, 40 hours per week, Monday–Friday 8:00 AM–4:30 PM.

Responsibilities include assessments, treatment planning, crisis intervention, and collaboration with families and agencies. Licensure requirements include LMSW/LPC-A/LMFT-A, with preference for LCSW/LMFT/LPC or licensed

Qualifications

• Master's Degree or higher in related field.
• Experience working with trauma, mental illness and/or substance use disorders preferred.
• Licensure/Certification/Registration: LMSW, LPC-A, LMFT-A required. LCSW, LMFT, LPC, Licensed Psychologist preferred. Driver's license required where transportation is needed.

Responsibilities

• Provides assessment and treatment interventions aligned with the program model.
• Conducts clinical and risk assessments.
• Collaborates with clients to create individualized, person-centered treatment plans.
• Demonstrates competency in age-related or specialty issues for the population served.
• Develops discharge plans utilizing natural supports.
• Maintains records according to agency policy.
• Provides crisis assessment and intervention during crises.
• May provide care in office, home, or community-based settings.
• Participates in supervision, consultations, and meetings.
