Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Case Manager to coordinate interdisciplinary care for designated patients and to optimize admission decisions and discharge planning. You will work with a team to achieve quality outcomes and maintain payor relationships, in a full-time, on-site role at our Pittsburgh-area hospital.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include OT, PT, RN, RT, SLP, or social worker.
• Current BLS certification required.
• Three to five years of inpatient experience is preferred.

Responsibilities

• Coordinate interdisciplinary care for a designated patient population.
• Review inpatient records to ensure appropriate level of care and discharge planning.
• Collaborate with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
