Job summary

Peninsula Post Acute is seeking a compassionate, organized Case Manager Assistant to join our team. This full-time role provides administrative support to the Case Management team and helps ensure residents and families receive timely, compassionate, and effective support.

The ideal candidate is detail-oriented, professional, and comfortable communicating with residents, families, healthcare professionals, and interdisciplinary team members.

Qualifications

• Healthcare experience or related field preferred.
• Strong organizational and communication skills.
• Detail-oriented with the ability to manage multiple priorities.
• Proficiency with computers and standard office applications.
• Ability to communicate professionally with residents, families and care teams.

Responsibilities

• Assist the Case Manager with daily administrative and coordination tasks.
• Support communication with residents, families, healthcare providers, and other care team members.
• Assist with discharge planning and coordination of services.
• Help gather and maintain accurate documentation and records.
• Assist with referrals, appointments, transportation, and other post-discharge needs.
• Coordinate information between departments and outside healthcare providers.
• Follow up on outstanding items to help ensure timely completion.
• Maintain confidentiality and comply with HIPAA and facility policies.
• Provide compassionate and professional customer service to residents and families.
• Perform other duties as assigned to support the Case Management department.
