Peninsula Post Acute in Burlingame, CA is seeking a compassionate Case Manager Assistant to join our team. This full-time role offers a competitive hourly rate and benefits, with a pay rate of $30.00-$35.00 per hour based on experience.

The assistant will provide administrative and coordination support to the Case Management team, assist with discharge planning, maintain records, and liaise with residents, families, and healthcare providers to ensure timely, compassionate service.

Qualifications

• Previous experience in healthcare, skilled nursing, rehabilitation, case management, social services, or a related field preferred
• Strong organizational and communication skills
• Excellent attention to detail and follow-through

Responsibilities

• Assist the Case Manager with daily administrative and coordination tasks
• Support communication with residents, families, healthcare providers, and other members of the care team
• Assist with discharge planning and coordination of services
• Help gather and maintain accurate documentation and records
• Assist with referrals, appointments, transportation, and other post-discharge needs
• Coordinate information between departments and outside healthcare providers
• Follow up on outstanding items to help ensure timely completion
• Maintain confidentiality and comply with HIPAA and facility policies
• Provide compassionate and professional customer service to residents and families
• Perform other duties as assigned to support the Case Management department

Position Overview

The Case Manager Assistant will provide administrative and coordination support to the Case Management team while helping ensure residents and their families receive timely, compassionate, and effective support throughout their stay.

The ideal candidate is highly organized, detail-oriented, professional, and comfortable communicating with residents, families, healthcare professionals, and interdisciplinary team members.

Key Responsibilities

• Assist the Case Manager with daily administrative and coordination tasks
• Support communication with residents, families, healthcare providers, and other members of the care team
• Assist with discharge planning and coordination of services
• Help gather and maintain accurate documentation and records
• Assist with referrals, appointments, transportation, and other post-discharge needs
