Job summary

Pacific Coast Post Acute in Salinas, CA is seeking a skilled Case Manager (RN preferred) to develop and implement patient care plans. You will work with residents, families, physicians, insurers and social services to deliver extraordinary care while managing costs and resources.

Responsibilities include coordinating admissions and discharges, reviewing plans of care, performing clinical assessments, ensuring timely documentation, and communicating plans to the care team at daily standups.

Qualifications

• Bachelor's degree in Nursing or Social Work is required.
• RN license preferred; LVN/LPN acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs and benefits.
• PCC knowledge preferred.
• Certificate as CCM a plus.
• Must maintain continuing education/licensing.
• Must remain in good standing with the Department of Public Health.

Responsibilities

• Develop and implement a plan of care with residents, families and the care team.
• Communicate regularly with residents and families about plans of care and treatments.
• Attend morning stand-up meetings to coordinate with the team.
• Coordinate admissions and discharges; manage referrals.
• Perform clinical assessments and direct coordination of medical services.
• Complete documentation in a timely manner.
• Coordinate care with providers, facilities, and third-party payers.
• Maintain professional interactions with residents, families, staff and partners.
