Job summary

Peninsula Post Acute is seeking an Admissions Coordinator to ensure smooth, welcoming admissions for new residents and families. This role serves as the main contact with hospitals, referral sources, and interdisciplinary teams to coordinate timely processes with confidentiality and compassion.

The ideal candidate will be highly organized, detail-oriented, and professional in a fast-paced healthcare environment, with experience in admissions and knowledge of Medicare/Medicaid processes.

Qualifications

  • Experience in admissions, healthcare, skilled nursing, long-term care, or related setting is preferred.
  • Knowledge of Medicare, Medicaid, insurance verification, and authorization processes is preferred.
  • Experience working with hospitals, referral sources, case managers, or healthcare providers is a plus.

Responsibilities

  • Serve as the primary point of contact for prospective residents, families, hospitals, physicians, case managers, and referral sources.
  • Coordinate and schedule resident admissions, including pre-admission assessments and required documentation.
  • Maintain accurate and up-to-date records in accordance with federal, state, and facility regulations.
  • Assist with insurance verification, authorizations, Medicare/Medicaid processes, and financial eligibility requirements.
  • Provide facility tours and communicate information about services, programs, and the admission process to prospective residents and families.
  • Collaborate with nursing, social services, business office, and administrative staff to ensure a seamless admission experience.
  • Prepare, organize, and distribute admission packets and required documentation.
  • Follow up with hospitals, physicians, case managers, and other referral sources to ensure timely and complete transfer of information.

Admissions Coordinator

Peninsula Post Acute is seeking a compassionate, organized, and motivated Admissions Coordinator to join our team.

Pay: $29.00-$30.00 per hour

Schedule: Friday - Sunday

Benefits: Comprehensive benefits package available

General Purpose

The Admissions Coordinator plays a vital role in ensuring a smooth, welcoming, and efficient transition for new residents and their families. This position serves as a key point of contact throughout the admissions process and works closely with hospitals, referral sources, residents, families, and interdisciplinary teams to coordinate timely and compassionate admissions.

Essential Duties and Responsibilities
  • Serve as the primary point of contact for prospective residents, families, hospitals, physicians, case managers, and referral sources.
  • Coordinate and schedule resident admissions, including pre-admission assessments and required documentation.
  • Maintain accurate and up-to-date records in accordance with federal, state, and facility regulations.
  • Assist with insurance verification, authorizations, Medicare/Medicaid processes, and financial eligibility requirements.
  • Provide facility tours and communicate information about services, programs, and the admission process to prospective residents and families.

  • Perform other duties as assigned.
Qualifications
Education and Experience
  • Prior experience in admissions, healthcare, skilled nursing, long-term care, or a related setting is strongly preferred.
  • Knowledge of Medicare, Medicaid, insurance verification, and authorization processes preferred.
  • Experience working with hospitals, referral sources, case managers, or healthcare providers is a plus.
