Job summary

Kern Regional Center is seeking a dedicated Service Coordinator/Case Manager to coordinate services for individuals with developmental disabilities in Kern, Inyo and Mono Counties. You will develop and implement IPP/PCP, prepare progress reports and annual reviews, and advocate for consumers and families.

The role requires strong communication, organization, and computer skills, plus the ability to seek resources and coordinate vendored services as needed.

Qualifications

• BA/BS degree in human services or related field and two years of relevant experience.
• Spanish/English bilingual is highly desirable.
• Detail-oriented and highly organized with strong communication skills.
• Valid driver's license with reliable transportation.

Responsibilities

• Provide overall service coordination, advocacy and services arrangement for consumers and families.
• Develop and implement IPP and PCP, progress reports and annual reviews.
• They will monitor consumer progress and program effectiveness; follow-up and reassess as needed.
• They may initiate vendored services and monitor service delivery; ensure compliance with regulations.
