Job summary
Kern Regional Center is seeking a dedicated Service Coordinator/Case Manager to coordinate community-based services, advocate for consumers, and develop IPPs and PCPs for individuals with developmental disabilities. This role covers consumers in traditional and Self-Determination programs, with emphasis on progress tracking and timely service delivery.
The ideal candidate will be bilingual (Spanish/English), highly organized, and proficient in standard software, with strong communication skills
Qualifications
- BA/BS degree preferred in human services or related field and two years of experience with developmental disabilities.
- Bilingual Spanish/English highly desired.
- Detailed-oriented and highly organized.
- Strong written and verbal communication skills; proficient with standard software.
Responsibilities
- Provide overall service coordination, advocacy and services arrangement for consumers and families.
- Develop and implement IPP/PCP and progress reports; conduct annual reviews.
- Coordinate follow-up, reassessments and periodic progress reviews as needed.
- Identify and connect with community resources; information and referrals; arrange vendored services.
