You will assess needs, develop individualized service plans, and work with multidisciplinary teams to deliver client-centered services.

The role emphasizes recovery, independence, and collaboration with therapists, clients, and families. Strong communication and knowledge of community resources are essential.

Qualifications

• Minimum of 3 years in behavioral health or related field preferred.
• Strong interpersonal communication with clients, families, and professionals.
• Knowledge of behavioral health practices and community resources.
• Ability to work independently and as part of a team.
• CPR certification preferred; NPI/Ohio Medicaid ID required per instructions.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.
• Manage case assignments, draft service plans, and review case progress regularly.
• Advocate for clients in decisions about clinical status and treatment options.
• Collaborate with therapists, clients, and family to meet client behavioral health needs.
• Facilitate clients in achieving independence and daily living skills.
• Perform other duties as assigned; uphold documentation and billing per CARF, CMS.
