Community Options, Inc. in Greenville, SC is seeking a Family Model Provider to open your home to an individual with developmental and/or physical disabilities.

This independent contracted role offers a safe, family-like setting 24/7 and per diem reimbursement plus monthly room and board. You will provide essential support, build a genuine relationship with the individual, and complete required trainings through Community Options.

Qualifications

• High School Diploma or GED required.
• Valid driver's license required.
• Previous Direct Support Professional experience is required.
• Must complete trainings through Community Options.
• Must be open to supervisory visits in your home.

Responsibilities

• Provide a safe, comfortable home for an individual with disabilities.
• Establish a supportive, genuine relationship with the person served.
• Offer 24/7 family-like housing and ensure basic physical and emotional needs are met.

We are seeking someone in the Greenville, SC area, who is available to provide a safe and comfortable home, while ensuring the basic physical and emotional needs required for our individuals to live a high-quality life. You must have the ability to establish a comfortable, supportive, and genuine relationship with the individual you house and support. This independently contracted position will provide a 24/7 family-like setting for an individual.

REQUIREMENTS:

• High School Diploma/GED
• Valid Driver's License
• Previous experience working as a Direct Support Professional is required
• Will need to successfully complete trainings through Community Options
• Must be open to supervisory visits in your home

Our Family Model Providers are contracted and reimbursed at a per diem rate PLUS they receive monthly room and board from the individual they support – this income is tax exempt.
