Job summary

Community Options, Inc. seeks Foster Care/Host Home Provider in Cookeville, TN. This independently contracted role offers a 24/7 family-like setting in your home for an individual with developmental and/or physical disabilities.

You must provide a safe, supportive environment and meet basic physical and emotional needs. We require a high school diploma or GED, a valid driver's license, prior Direct Support Professional experience, and completion of internal trainings.

Qualifications

• High School Diploma or GED required.
• Valid drivers license required.
• Previous experience as a Direct Support Professional is required.
• Must successfully complete trainings through Community Options.
• Must be open to supervisory visits in your home.

Responsibilities

• Provide a safe, comfortable home for the individual you support.
• Establish a positive, supportive relationship with the individual.
• Open your home to provide 24/7 family-like setting.
• Ensure basic physical and emotional needs are met for a high-quality life.

This independently contracted position will provide a 24/7 family‑like setting for an individual.

We are looking for someone in the Cookeville, TN area!

We are seeking someone who is available to provide a safe and comfortable home, while ensuring the basic physical and emotional needs required for our individuals to live a high‑quality life. You must have the ability to establish a comfortable, supportive, and genuine relationship with the individual you house and support.

REQUIREMENTS:

• High School Diploma/GED
• Valid Driver's License
• Previous experience working as a Direct Support Professional is required
• Will need to successfully complete trainings through Community Options
• Must be open to supervisory visits in your home

Our Host Home Providers are contracted and reimbursed at a per‑diem rate PLUS they receive monthly room and board from the individual they support – this income is tax exempt.
