Job summary

Community Options, Inc. is seeking caring individuals to become Family Model Providers in the Chattanooga, TN area. You will open your home to a person with developmental or physical disabilities and provide a safe, supportive environment with ongoing daily care.

This independent contract role offers a per diem rate plus monthly room and board, with income that is tax exempt. Applicants must have a high school diploma or GED, a valid driver’s license, and prior experience as a Direct Support

Qualifications

• High School Diploma/GED required.
• Valid Driver's License required.
• Previous experience as a Direct Support Professional.
• Must complete trainings through Community Options.
• Must be open to supervisory visits in your home.

We are seeking someone in the Chattanooga, TN area, who is available to provide a safe and comfortable home, while ensuring the basic physical and emotional needs required for our individuals to live a high-quality life. You must have the ability to establish a comfortable, supportive, and genuine relationship with the individual you house and support. This independently contracted position will provide a 24/7 family-like setting for an individual.

REQUIREMENTS:

• High School Diploma/GED
• Valid Driver's License
• Previous experience working as a Direct Support Professional is required
• Will need to successfully complete trainings through Community Options
• Must be open to supervisory visits in your home

Our Family Model Providers are contracted and reimbursed at a per diem rate PLUS they receive monthly room and board from the individual they support – this income is tax exempt.
