Job summary

Community Options, Inc. in Nashville, TN seeks a Family Model Provider to open their home to a person with developmental or physical disabilities, providing safety and emotional support in a family-like setting.

This independent contract involves 24/7 care and building a genuine, trusting relationship with the individual. Requirements include a high school diploma or GED, valid driver's license, prior Direct Support Professional experience, and willingness to undergo Community Options trainings

Qualifications

• Must have High School Diploma or GED.
• Previous DSP experience required.
• Must complete Community Options trainings and allow supervisory visits.

Responsibilities

• Provide a safe, comfortable home for the individual.
• Establish a supportive, genuine relationship with the person served.
• Adhere to Community Options training and supervisory processes.
• Operate as an independently contracted provider in a 24/7 family-like setting.

We are seeking someone in the Nashville, TN area, who is available to provide a safe and comfortable home, while ensuring the basic physical and emotional needs required for our individuals to live a high-quality life. You must have the ability to establish a comfortable, supportive, and genuine relationship with the individual you house and support. This independently contracted position will provide a 24/7 family-like setting for an individual.

REQUIREMENTS:

• High School Diploma/GED
• Valid Driver's License
• Previous experience working as a Direct Support Professional is required
• Will need to successfully complete trainings through Community Options
• Must be open to supervisory visits in your home

Our Family Model Providers are contracted and reimbursed at a per diem rate PLUS they receive monthly room and board from the individual they support – this income is tax exempt.
