Qualifications

• Bachelor’s Degree in Social Work or related Human Service discipline required.
• Two years’ experience working with youth at risk and families.
• Experience in Child Welfare.
• Ability to work with culturally diverse families.

Responsibilities

• Complete comprehensive assessments of children and families to identify strengths, needs, risks, and service recommendations.
• Develop, implement, monitor, and update individualized service and permanency plans with the team.
• Maintain regular contact with children, families, foster parents, and service providers per policy.
• Coordinate and monitor visitation between children and family members and assess progress toward permanency goals.
• Lead or participate in treatment team meetings, case conferences, and permanency planning reviews.
• Provide case management guidance and resource coordination to support safety and permanency outcomes.
• Coordinate with schools, healthcare providers, and community services to ensure needed supports.
• Maintain case documentation and ensure compliance with regulations and agency policy.
• Attend Family Court proceedings; prepare reports and testify when required.
• Serve as a liaison among CHJC, local departments of social services, courts, and community partners.
