The Children’s Home of Reading is seeking a Foster Care Case Manager to support children and families in the Reading, PA area. This full-time role involves intake processing, service plan development, ongoing coordination, and direct client contact with the goal of promoting independence and stability.

Responsibilities include maintaining caseloads, coordinating with agencies, transporting clients, and providing crisis intervention as needed.

Qualifications

• Bachelor’s degree in social work, psychology, or related field.
• One year of verified experience in case management or counseling with children and families (drug and alcohol experience preferred).
• Three written references demonstrating past educational or employment experience.
• Must be at least 21 years of age.
• Valid PA driver’s license with good driving record and auto insurance.
• PA Act 33/34 clearances and DPW FBI clearance.
• Pre-employment physical from a licensed physician.
• USCIS employment eligibility per 8 U.S.C. 1324a.
• Availability and willingness to work a flexible schedule.

Responsibilities

• Ensures training requirements are completed and reported to the Program Manager.
• Completes intake for new clients and writes individualized service plans.
• Orients new youth and families to SafeGuards programming.
• Maintains a caseload of 12 to 15 children and foster families.
• Maintains contact with referring agency weekly and coordinates activities.
• Transports clients to visits and other locations as needed.
• Provides crisis intervention as needed and maintains confidential records.
