Job summary

CHOR Youth & Family Services in Reading, PA is seeking a dedicated Case Manager for its foster care program. This full-time role involves managing a caseload of 12–15 children, coordinating service plans, and maintaining regular contact with families and referring agencies.

Qualified candidates will have at least 1 year of case management or counseling experience with children and families, plus required clearances, a valid PA driver’s license, and willingness to travel as needed.

Qualifications

• One year verified experience in case management or counseling with children and families.
• Three written references demonstrating past educational or employment experience.
• Must be at least 21 years of age.
• Valid PA driver’s license with good driving record and auto insurance.
• PA Act 33/34 clearances and DPW FBI clearance.
• Pre-employment physical confirming fitness for work with children.

Responsibilities

• Completes intake for new clients and writes individualized service plans.
• Maintains a caseload of 12 to 15 children and their foster families.
• Maintains regular contact with referred agencies and coordinates services.
• Transports clients to visits and locations as needed.
• Provides crisis intervention as required and maintains confidential records.
• Supports program operations and adherence to HIPAA standards.
