Job summary

One Hope United is seeking a dedicated Case Manager in Florida to provide direct case management to children in foster care and their families. You will collaborate with therapists, foster parents, and school professionals to implement case plans and coordinate services in home or community settings.

The role requires strong documentation, confidentiality, and ability to participate in trainings, court proceedings, and team meetings.

Qualifications

• Bachelor’s degree in a qualifying field required; master’s degree in relevant fields preferred
• CWCM certification preferred; willingness to obtain CWCM within 1 year of employment
• Must have reliable transportation and ability to drive daily for work

Responsibilities

• Provides direct case management services to children, birth families, foster families and primary caregiver in home, daycare, preschool or school settings
• Develops, documents and monitors case plans; coordinates services and maintains confidentiality
• Maintains face-to-face and telephone contact with assigned children/families and completes required documentation
• Attends and participates in trainings, staffing reviews, and court-related activities as required
• Links families to community resources and advocates for service needs
• Tracks court dates and prepares documentation for court and lead agency
• Maintains caseload productivity and timely case plan updates
• Performs crisis intervention and ensures safety of child during crises
