Job summary

Presbyterian Children’s Homes and Services (PCHAS) is seeking a Foster Care Case Manager to provide safety and services to children and families in foster care. You will coordinate individualized services across settings and document support activities to promote permanency.

The role requires managing a caseload, conducting monthly home visits, and facilitating Family Support Team meetings. A commitment to compassionate child welfare is essential.

Qualifications

• Bachelor’s degree in a related human services field is required.
• Master’s degree preferred for advancement.

Responsibilities

• Follows agency policies and delivers seamless services to children and families.
• Maintains a caseload of 12 to 15 children.
• Provide full case management from assignment to case closure.
• Document activities in state and consortium databases (FACES, CSWIS).
• Maintain monthly face-to-face contact with each child and family.
• Attend court hearings and facilitate Family Support Team meetings.
• Participates in FCCM on-call rotating schedule.
