Humankind Foster Care is seeking a dedicated Treatment Foster Care Case Manager in Lynchburg, VA. This full-time role involves documenting, assessing needs, conducting home visits, and coordinating meetings to support foster children and families.

Qualifications

• A baccalaureate degree in any field plus two years of experience in providing casework services to children and families.
• Or a baccalaureate degree in social work or a related field with one year of casework experience.
• Or a master's/doctorate in social work with a placement or one year of casework experience.

Responsibilities

• Complete and maintain thorough documentation including monthly notes and reports, 14/30/45 day plans, social histories, and reports.
• Assess foster children's needs and implement objectives, strategies and services.
• Complete home and field visits multiple times a month for each child.
• Attend or coordinate meetings for foster children and families (FAPT, Court, FPM, Treatment Teams, VEMAT).
• Supervise visitations and provide transportation; make referrals to agencies.
• Participate in trainings required by LCPA standards, HumanKind, and supervisor.
• Participate in audits and licensing inspections.
• Use placement/matching protocol when placement assistance is needed.
