Job summary

The Villages of Indiana is seeking a Case Manager – Intensive Foster Care to lead intake, assessments, and treatment planning for children in foster care. You will collaborate with a multidisciplinary team to coordinate services and support foster families.

As the main liaison with the DCS FCM, you will attend CFTM meetings, provide training and skill-building for families, and help secure respite and enrichment activities.

Qualifications

• Bachelor’s from an accredited four-year college.
• If BSW, then LBSW is needed.
• Five (5) years of supervised, paid casework experience; two (2) years in an approved child placing agency.

Responsibilities

• Handle intake and implement assessments to develop Treatment Plan and Family Support Plan.
• Coordinate services with team members to meet client needs.
• Be the main contact with the DCS FCM and attend CFTM meetings as needed.
• Collaborate with foster parents to provide support, trainings and support groups; assist with respite and extracurricular activities; assist with behavior management and skill building.
