Qualifications

• Must be 21+ years of age.
• LSW or LPC or Bachelor's degree in human services.
• Bachelor's degree with field experience of 2 years; WV wording.
• Bachelor's or Master's degree with 7 hours in behavioral sciences; substitution rule.

Responsibilities

• Participate in referral matching for children and families; advocate for best home match.
• Develop and monitor each child’s Initial and Comprehensive Treatment Plans.
• Complete assessments (Biopsychosocial, Independent Living, Diagnostic).
• Refer foster child to support services (behavioral health, independent living).
• Ensure initial legal documentation is obtained prior to placement or service delivery.
• Ensure safety plans are developed during intake.
• Create a plan and monitor for IT P execution and updates.
• Coordinate with foster parents, agencies, and schools to meet education goals.
• Maintain confidentiality and comply with state licensure and accreditation requirements.
