Job summary

Presbyterian Children’s Homes and Services (PCHAS) is seeking a Foster Care Case Manager to safeguard children and support families in foster care. The role requires coordinating individualized care plans, facilitating Family Support Team meetings, and linking services across home, school, and community settings.

You will maintain caseloads, document progress in state systems, and attend court hearings as needed. Travel may be required to various home and community locations.

Qualifications

• Must possess a bachelor’s degree in social work, psychology, counseling, education, criminal justice, or a related human services field.
• Master’s degree preferred.
• Minimum of two (2) years of relevant case management experience.

Responsibilities

• Follows all contractual and agency policies, procedures, and seamless delivery of services to children and their families.
• Maintains a caseload of 12 to 15 children depending on program capacity.
• Provide full case management services to MO foster/adoptive care system from assignment until case closure or transfer.
• Documents all activities and maintain data entry in state databases in a timely manner.
• Has face-to-face contact with each assigned child and family at least monthly.
• Evaluate and report impact of services to team and judiciary; attend court hearings.
