Job summary

Sevita, through Pennsylvania Mentor, provides foster care and related services to youth facing various placements and transitions. The Case Manager Foster Care role drives development and review of services, ensuring quality care and collaboration with families and external managers.

A Bachelor’s degree and 2+ years of case management are required. We offer a full compensation/benefits package, 401(k) with company match, paid time off and holidays, and opportunities for nationwide advancement.

Qualifications

• Bachelor's degree in social work, criminal justice, or education.
• At least 2 years' case management experience, including at least 1 year working with at-risk youth.
• Valid driver's license, current car registration, and auto insurance.
• Licensure and training as required by state (CPR, First Aid, Behavioral Intervention Techniques, etc.).
• A reliable, compassionate attitude and commitment to quality.

Responsibilities

• Coordinate development, review, and implementation of services, case management, behavior intervention plans, and treatments.
• Oversee all services provided to individuals and families, monitor quality and regulatory compliance.
• Assist in developing quality services and engaging activities tailored to individual needs.
• Build and maintain relationships with families and external case managers.
