Job summary

Auberle in McKeesport, PA seeks a Foster Care Case Manager to build trusting relationships with families and children in resource homes. You'll conduct home visits, develop individualized care plans, and coordinate with families and service providers to ensure safe, stable, and supportive outcomes.

The role requires flexibility, ongoing documentation, and on-call crisis support, with travel across the greater Pittsburgh area and training at the McKeesport campus.

Qualifications

• Act 33/34 and FBI Clearances required.
• Bachelor's degree preferred, HS diploma/GED required, related experience & lived experience may be accepted.

Responsibilities

• Connect, develop, and maintain a strong, trusting relationship with families served and children placed to foster engagement and support growth.
• Conduct regular home visits to monitor the child’s progress and well-being.
• Develop and implement individualized care plans.
• Coordinate with natural families, resource families, and other service providers to promote open communication and collaboration for comprehensive care.
• Conduct regular meetings with children, caregivers, and professionals in various settings (homes, community spaces, offices).
• Maintain detailed, accurate, and timely documentation for safety and reporting.
