Job summary

Necco seeks a Foster Care Case Manager to support children and foster families across office, school, and home settings. You will help develop treatment plans, coordinate services, and ensure compliance with licensing standards.

The role requires strong communication, travel to placements, and ability to navigate multiple systems, including EHRs, while maintaining confidentiality and safety.

Qualifications

• Must be 21 years of age.
• LSW/LPC or BA in human services (OH, KY, GA).
• BA with field experience (WV).
• BA or MA with behavioral sciences hours (social work, psychology, sociology).
• 1 year of casework can substitute for the 7 hours.
• 2+ years experience with children with emotional/behavioral problems.
• Liability auto insurance 100/300/100.
• Valid driver’s license.
• Strong organizational and written/oral communication skills.
• Proficient with MS Office, databases, EHR, dashboards.

Responsibilities

• Participate in referral matching for children and families.
• Develop and implement Initial/ Comprehensive Treatment Plans.
• Coordinate and complete assessments (biopsychosocial, independent living, diagnostic).
• Refer foster child to applicable support services (behavioral health, independent living).
• Ensure initial legal documentation is obtained before placement.
• Ensure safety plans are developed during intake.
• Develop Individualized Treatment Plans (ITP) and monitor progress.
• Support foster parents and child with plan implementation and tracking.
• Maintain confidentiality and meet state/licensure requirements.
• Travel to placements as essential and provide training to families.
• Maintain and document service meetings with stakeholders.
• Document and manage medication logs and aftercare referrals.
