Job summary

Presbyterian Children’s Homes and Services (PCHAS) is seeking a Foster Care Case Manager to provide safety and services to children and families in foster care within Missouri. The role requires coordinating individualized services across settings and documenting support activities to promote permanency.

Responsibilities include facilitating Family Support Team meetings, developing care plans, and ensuring effective utilization of services across home, school, and community environments.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, education, criminal justice, or related field.
• Master’s degree preferred.
• Minimum of two years of relevant case management experience.

Responsibilities

• Maintain a caseload of 12 to 15 children depending on program capacity.
• Provide full case management services to children and families in MO foster/adoptive care.
• Document activities and data entry in FACES and CSWIS in a timely, accurate manner.
• Maintain monthly face-to-face contact with each assigned child and family; regular telephone contact for progress reports.
• Coordinate and facilitate Family Support Team meetings and consent to court hearings as needed.
