Job summary

Cornerstones of Care is seeking a Foster Care Case Management Specialist to join our team in Missouri. You will exercise discretion and independent judgment in managing services for children on their caseload, aiming for safety and permanency, coordinating activities within the Family Support Team, and moving children toward a permanent home.

As a MO Foster Care team member, you will work with 7 other team members and report to our Manager of Foster Care Case Management.

Qualifications

• Bachelor's degree in social work, criminal justice or related human services field is required.
• Master's degree and case management experience preferred.
• At least 21 years of age and pass background check, drug screening.

Responsibilities

• Manage and provide services to children and families referred under the Foster Care Case Management Contract.
• Continually analyze and assess each family and child situation to develop recommendations for support services and resources.
• Facilitate service planning with the child, family, and Family Support Team (FST) per policy.
• Coordinate supportive services to ensure timely and efficient service delivery.
• Assist the family in cultivating natural and informal supports to achieve safety and permanency.
