Pchas is seeking a Foster Care Case Manager in St. Louis, Missouri to ensure safety and services for children and families in foster care. You will facilitate Family Support Team meetings and provide case management services to children from the time of assignment until closure.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, education, criminal justice, or related field required.
• Preferred master’s degree with at least two years of relevant case management experience.
• Ability to navigate multiple data entry systems.

Responsibilities

• Facilitates Family Support Team meetings and develops individualized care plans.
• Maintains a caseload of 12 to 15 children and provides full case management services.
• Has face-to-face contact with each assigned child and family at least monthly.
