Job summary

Presbyterian Children’s Homes and Services (PCHAS) seeks a Foster Care Case Manager to oversee safety and services for children in foster care. You will facilitate Family Support Team meetings, develop individualized care plans, and coordinate services across home, school, and community settings.

You will maintain monthly face-to-face contact, document activities in state databases, and testify in court as needed.

Qualifications

• Bachelor’s degree required; master’s degree preferred.
• Minimum of two years of relevant case management experience with a bachelor’s degree or one year with a master’s degree.

Responsibilities

• Follows all agency policies and ensures timely data entries for court reports and performance measures.
• Maintains a caseload of 12–15 children; adjusts with agency needs.
• Provides reasonable efforts to families as outlined in policy.
• Supports placement stability and coordinates resources, including after-hours crises.
• Documents activities and maintains records in state CSWIS and/or FACES systems.
• Maintains monthly face-to-face contact with each child and family; regular phone updates with providers.
• Evaluates impact of services and testifies at court hearings when required.
• Participates in FCCM consortium on-call schedules.
