Regional Center of the East Bay in Concord or San Leandro, CA seeks a Case Manager II (Forensic Assessment Counselor) to oversee the full case management process for individuals with developmental disabilities. The role includes assessment, planning, advocacy, monitoring, and coordination of services to ensure rights and access to supports.

The position requires a Master’s Degree in Social Work, Psychology or a related field, strong communication skills, Spanish language ability, and a valid

Formación

• Master’s Degree in Social Work, Psychology or related field.
• Excellent written and verbal communication skills.
• Ability to speak and read in Spanish and hold a valid driver’s license.

Responsabilidades

• Facilitate the planning process and develop the IPP/IFSP for assigned Consumers and oversee the implementation of the IPP/IFSP.
• Coordinate and support the implementation of the Consumer's plan including identifying supports and evaluating funding.
• Advocate on behalf of consumers to protect their rights and ensure access to services.
• Assist the consumer and family in obtaining essential diagnoses and evaluations from appropriate practitioners.
• Monitor the consumer’s health and safety.
• Maintain documentation and contacts as required by law, regulation and policy.
• Implement organizational policies and procedures.
• Complete, in a timely manner, functional and/or developmental reviews.
• Keep supervisor informed of caseload status and unmet needs in the community.
• Perform other duties as assigned.
