Circle of Life is seeking a Float Registered Nurse Case Manager (QP) to support Waite Park, MN and surrounding areas. You will travel to provide direct nursing care in patients' homes, schedule visits, and collaborate with an interdisciplinary team to deliver high-quality services.

The role includes supervising caregivers and ensuring care plans are followed, with the opportunity to be involved in staffing and onboarding to support growth in the Central Region and Twin Cities area.

Qualifikationen

• Valid MN RN license required.
• Willingness to travel for home visits in Twin Cities and surrounding areas.
• Experience in case management or home care is a plus.

• Provide 1:1 nursing services in patients' homes, traveling as needed.
• Coordinate visits, onboarding, and staffing coverage for the region.
