Job summary

AlignMed Partners is seeking a Full-Time Advanced Practice Provider (NP/PA) to deliver primary and urgent care across three Baltimore, MD centers: Franklin Woods Center, Advanced Rehab at Autumn Lake, and Hammonds Lane Center. The role offers a 40-hour week with no on-call or weekend requirements.

The base salary ranges from $115,000 to $125,000 per year, plus an incentive compensation program with no cap. New grads are welcome to apply, with relocation and comprehensive benefits available.

Qualifications

• The Advanced Practice Provider must be a graduate from an accredited School of Nursing with RN licensure and an MSN, specializing in Older Adult or Family (FNP).
• The PA must be a graduate from an ARC-PA accredited program and pass the PANCE to obtain PAC designation; PA licensure required; NCCPA certification.
• A minimum of two years of adult/geriatric experience is preferred.

Responsibilities

• Provide primary and urgent care services to nursing center residents in collaboration with other medical providers.
• Collaborate on-site with Center Leadership to achieve center goals and priorities.
• Coordinate with Medical Director/Attending Physician for patient-specific medical care.
