Qualifications

• Bachelor's in Pharmacy or Pharm.D. from ACPE-accredited program.
• No experience required; PGY-1 or equivalent preferred.
• Virginia State Board of Pharmacy license (or within 60 days); BLS required.
• Physical: prolonged sitting, typing; lift <20 lbs; exposure to hazards.

Responsibilities

• Provide clinical pharmacy services across inpatient units per needs.
• Review and manage medication orders prioritizing safety and efficacy.
• Serve as clinical resource to physicians, nurses, and care teams.
• Identify and resolve complex medication-related problems.
• Support transitions of care with reconciliation and recommendations.
• Maintain continuity of pharmacy services across units and shifts.
