Job summary

UVA Health is seeking a Float Clinical Pharmacist to deliver clinical and operational support across multiple inpatient service lines, ensuring safe, effective medication use and high-quality care for diverse patient populations.

You will apply broad acute care knowledge while expanding familiarity with varying specialties, collaborating with care teams and contributing to transitions of care. This role thrives on adaptability and continuous learning in a premier academic setting.

Qualifications

• Education: Bachelor's in Pharmacy or Pharm.D. from ACPE accredited program.
• Licensure: Virginia State Board of Pharmacy license or eligibility within 60 days.
• AHA Health Care Provider BLS certification required.
• PGY-1 residency or equivalent clinical experience preferred.

Responsibilities

• Provide clinical pharmacy services across multiple inpatient areas based on care needs.
• Review, verify, and manage orders with emphasis on safety and optimization.
• Serve as a clinical resource to physicians, APPs, nurses, and care teams.
• Identify and resolve medication-related problems for patients with complex conditions.
• Support transitions of care through reconciliation and patient-specific recommendations.
• Maintain continuity of pharmacy services across units and shifts.
