We are searching for a Certified Nursing Assistant who is interested in home services opportunities.

Responsibilities

• Implement treatment and plan of care, using sound judgment and critical thinking
• Maintain accurate, detailed, and timely documentation consistent with the plan of care
• Perform all skills and procedures to the scope of practice as defined in and adhere to the policies, procedures, and protocols of the agency of employment

Requirements

• Must be able to travel all surrounding hours. Drive time is only included if going an hour or more away, otherwise, mileage over 30 applies
• must be available to flex time
• no preferences of clients
• on call one weekend a month. Lead CCM will assign that weekend in the rotation; it reflects on the schedule.
• PTO has to be submitted all PTO time. Drive time is only included if going an hour or more away and will be on their schedule; otherwise, mileage over 30 applies
• Physical within the last yearand negative TB skin test or chest x-ray
• Licensed driver
• Current CPR certification
• Proof of eligibility to work in the US.
• Reliable car / auto insurance.
• Clean background and criminal record.
• Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds
