Job summary
AdventHealth in Florida seeks an experienced Registered Nurse for home health case management. You will assess patients, coordinate care, and deliver nursing services in patient homes across Hillsborough, Pasco, Pinellas, Polk and surrounding areas.
The role requires an RN license, BLS, and at least 1 year of clinical nursing experience; a driver’s license and auto insurance are also needed for home visits. Competitive pay and benefits offered.
Qualifications
- RN license required in Florida.
- 1+ relevant clinical nursing experience.
- Home health case-management experience preferred.
- Medicare-certified home health knowledge preferred.
Responsibilities
- Evaluate home environments for safety and infection control.
- Review patient history and lab data; coordinate care with the team.
- Provide skilled nursing care and prescribed treatments in home settings.
- Develop patient-specific care plans with patient, family, and physician.
- Maintain clinical records and care plan updates.
- Optimize daily schedules for productivity and best-practice visits.
