Job summary

Team Select Home Care is seeking a Home Health Aide (HHA) to provide skilled personal care and ADL support to patients at home under the supervision of a Registered Nurse. You will report to the RN and participate in care planning and documentation.

Responsibilities include direct patient care, medication assistance where allowed, and following agency policies. A HS diploma, 18+ years of age, CPR/health certifications, and flexible scheduling are required.

Qualifications

• High School Diploma or GED required per state regulations.
• Must be 18 years or older (where required).
• Maintains licensure or certification per state regulation.
• Maintains current CPR and health certifications as required.

Responsibilities

• Provides services as ordered in the plan of care and within state law.
• Assists with direct patient personal care, household tasks, meals, and other delegated duties.
• Performs simple procedures as an extension of therapy or nursing services as assigned.
• Assists with ambulation or exercises as directed.
• Assists with medications ordinarily self-administered as allowed by state regulations.
• Follows infection control practices, including handwashing and precautions per RN direction.
• Completes visit records in a timely manner per agency policy.
• Reports changes in patient condition to the RN promptly.
• Performs household services essential to health care in the home as assigned.
• Attends staff meetings and care conferences as scheduled.
