Job summary

Team Select Home Care is seeking a Home Health Aide (HHA) to provide skilled personal care and ADLs in patients’ homes under RN supervision. The role includes following the care plan, assisting with daily tasks, and documenting visits accurately.

The candidate should have a High School Diploma or GED, be at least 18, hold CPR certification, and maintain state-required certifications. We offer comprehensive benefits, flexible scheduling, weekly pay, and ongoing training.

Qualifications

• High School Diploma or GED is required
• 18 years of age or per state regulation
• Maintains licensure or certification per state regulation
• Maintains current CPR certification and health certifications per state requirements

Responsibilities

• Provides services as ordered in the plan of care and is permitted to perform under State law
• Performs direct patient personal care, household chores, meals, and other assigned tasks
• Assists with simple procedures as an extension of therapy or nursing services
• Assists in ambulation or exercises as assigned
• Assists with medications ordinarily self-administered as assigned per state regulations
• Maintains infection control practices and handwashing per RN directions
• Completes visit records timely per Agency policy
• Reports changes in patient condition to the RN
• Performs home health tasks including essential health care services
• Attends staff meetings and patient care conferences as scheduled
• Follows agency policy for cleaning equipment between patient uses
• Carries out, reports and documents care timely
• Does not accept untrained assignments
• Follows RN delegation tasks as applicable
• Maintains confidentiality of patient information
• Reports complaints, infections, variances, accidents, or unusual occurrences
• Participates in QAPI activities
• Maintains acceptable attendance per policy
• Maintains neat appearance and professional conduct
• Demonstrates time management and timely documentation
• Attends in-services and mandatory trainings
• Maintains clean work environment
• Demonstrates sound judgment and decision making
• Maintains human resource renewables as directed
• Performs other duties as assigned
